Registration Forms are available at the Council Service Center or by calling 405-840-1114 ext.249 or 1-888-841-1114. Download the registration form in Microsoft Word format. All staff and participants must have a current Class III Health Form. If you have a current health form from any of the high adventure bases (Sea Base, Northern Tier or Philmont), you may use that form. Download Class III Health form in PDF format.
